Institute of Medicine (IOM) Reports
In 1996, the Institute of Medicine (IOM) launched an ongoing effort, the IOM Health Care Quality Initiative, that focuses assessing and improving the quality of care in our nation. In this effort, the IOM published a series of books addressing various issues in health care. Two of these reports, To Err is Human and Preventing Medication Errors, address medical errors.
