Toolkits

The Medication Management Improvement System has been successfully implemented into two distinct environments - home health agencies and Medicaid waiver care management agencies.

The aim of the following toolkits is to provide home health agencies and community based care management agencies access to knowledge gained from past experience regarding implementing a medication management intervention into practice. The toolkits offer practical advice on the scope of the potential medication related problems amongst the elderly and suggest how agencies can begin addressing this very serious issue.
