HomeMeds: Medication Management Improvement System (MMIS) Replication Report
A comprehensive Replication Report was developed for the AoA's 2003 Evidence-Based Disease Prevention Grant. It provides an overview of the steps taken to adopt an evidence-based program and the lessons learned during this implementation process. Note: The report addresses the topics covered under the RE-AIM format (Reach, Effectiveness, Adoption, Implementation and Maintenance).
