Become A Site

Thanks to generous funding from The John A. Hartford Foundation, our goal now is to make medication management a national standard of practice in care management programs for frail elders. We will select at least four “early adopter” organizations to help us bring this innovative and effective program to other states. We will also be happy to assist other sites to implement the system.

Benefits to becoming an official demonstration site:

    1. Beyond the obvious benefits to clients and improved regulatory compliance on client safety, our demonstration sites will receive the following benefits:
    2. Comprehensive education and training for staff on medication use and problems among   older adults;
    3. Detailed manuals, protocols, and handouts on medication management;
    4. Use of a modestly-priced, secure on-line medication risk screening and management tool;
    5. Personalized consultation to adapt the intervention for your agency’s needs;
    6. Site support resources to help defray some of the initial costs; and
    7. National prominence as part of the vanguard in bringing this AoA evidence-based disease prevention program from its pilot phase in California to new states.

Because implementing medication management is a significant change in practice, we don’t recommend it for every agency. We have partnered with the National Council on Aging (NCOA) to create a tool that assesses an organization’s readiness to undertake this change. It is based on research that has shown that there are two dimensions to this readiness – the willingness of people at all levels of the organization to do what it takes to reach and sustain new goals and also the capacity in terms of resources and systems.

The readiness tool takes about 15 minutes to complete if you are prepared with certain information. After completing the tool, you will see a report on your results. Partners in Care will also receive the report and will contact you about your interest in the project. People find that completing the tool is well worth their time and that the report is very interesting and useful.
